What you need to know before your visit during the current phase of the COVID-19 Pandemic

We’ve been working behind the scenes to ensure we open a COVID-19 safe environment for our patients and staff, so there will be a few changes to how we work. It’s different, but not that different.

When You Call Us

  • We will be asking a few more questions than usual. These are some screening questions to assess your current health status.
  • We now have a new COVID-19 consent form online. It is easy to find on our website, www.foreversmile.co.uk/covid/consent and can be signed and sent to us at the click of a button. Please sign and send this form on the day of your appointment and not earlier.
  • No need for any more paperwork in the Practice. Don’t worry if the internet is not for you. We can fill the form in when you are here.

On Your Way Here

  • We would be grateful if you could wear a face covering to cover you nose and mouth.
  • If you are able to attend your appointment on your own please do so. If you have mobility issues and need a chaperone please let us know. We are working to minimize the number of people in the Practice at any one time to enable us to maintain physical distancing.
  • Try not to arrive more than 5 minutes early for your appointment.
  • Please do not travel or attend if either you or any member of your household has developed a persistent cough or high temperature since you signed our online consent form. Let us know and we will be very empathetic.

Coming In

  • We ask that you pop on your face covering and sanitize your hands as you enter the Practice.
  • Your temperature will be taken just to be sure. In accordance with medical guidance, if your temperature reads 37.8°C or higher you will be unable to proceed with your appointment and we will recommend that you be tested for COVID-19. We cannot retake your temperature.
  • You will be asked three questions on your arrival.
  • Have you or any of your household experienced a cough in the past two weeks?
  • Have you or any member of your household experienced loss of smell or taste?
  • If the answer to these questions is no and your temperature is normal we will proceed!
  • We’ve staggered our hours and appointments to limit the number of people in reception at all times. If there happens to be other patients in reception please maintain your distancing. We will do all we can to ensure distancing is maintained.
